How to Create & Edit Forms on Dubsado 3.0

Dubsado forms are a powerful and beautiful part of your client system. You can upload existing PDFs to share key information, or build fully custom forms to gather the exact details you need. Everything lives in one organized, easy-to-access spot right inside each client’s project portal - keeping the process clear for you and seamless for them.
You’ll receive email or browser notifications when forms are edited or submitted, so you always know what stage your client is in without needing to manually check in.
And don’t hold back on creativity - Dubsado forms can do so much more than basic questionnaires. You can embed images, video, smart fields, file uploaders, package selections, signatures, and more - making every form dynamic, intuitive, and tailored to your workflow.
Form templates power every stage of your client journey - from signed agreements to intake, proposals, and automated lead capture. Each form type has a specific role, and understanding the differences helps you structure your projects, trigger workflows, and gather the right information at the right time.
Form types, simplified:
Contract - The main project agreement (one per project). Connects to Proposal + Invoice for seamless conversion.
Sub-Agreement - Any additional document requiring a digital signature.
Questionnaire - Flexible forms for collecting client responses with multiple question types.
Proposal - The only form that displays package templates for selection. Can pre-populate contract smart fields and prepare invoices based on selections.
Lead Capture - The intake form for new prospects. Automatically creates a client + project, applies a workflow, and can trigger automations.
PDF - PDF files can also be uploaded to your Form Templates area in Dubsado and sent manually or via Automated Flows as a 'Form'
Every form plays a specific role, helping you guide clients smoothly through interest, decisions, signatures, intake, and onboarding - all in one central place.
To create a new Form Template, or to edit an existing one, follow these steps:
Click on "Forms" in the left sidebar
Ensure you're clicked into "Form Templates" though this should be selected by default
Click "+ New Form Template" to create a brand new form, or simply click into an existing form to make edits.
